If you provide computers, peripheral devices, network infrastructure, technology maintenance services, software license subscriptions, technology support services, software licenses, information systems consulting or information systems use the Information Systems link to download the form. Supply and Services Suppliers
If you provide commodity items and services, such as office supplies, print products, roll printing paper, construction work, facility services, records storage, promotional items, copiers and other items NOT related to information systems use the Supply and Service Suppliers link to download the form. Self Certification Letter Form
The U.S. Small Business Administration’s (SBA) self certification letter officially communicates a company’s size and status information to BlueCross. Additional documents must accompany claims of some statuses as directed in the form. Visit the sites provided on the Related Links page for more information.
